Individual Service Plan (ISP)

An Individual Service Plan (ISP) begins when an individual leaves/graduates from high school. This document picks up where the Individual Education Plan (IEP) leaves off and it is important document in adult planning for individuals with developmental disabilities. The school will begin to transition services prior to graduation or completion of high school.

The Individual Service Plan (ISP) is the written details of the supports, activities, and resources required for the individual to achieve personal goals. The Individual Service Plan is developed to articulate decisions and agreements made during a person-centered process of planning and information gathering. This document then outlines the services to meet the life goals of that individual as well as their support needs. It is from the ISP that enables the provision of Home and Community Based Services.
